Patient Survey 2018
Our Patient Survey has now been completed and the report is on our Website for you to read. Please note that from April 2019 there will be limited on the day appointments that you can book on a Tuesday and a Thursday from 6.45am until 8.00am until our telephone lines go over. This is to ensure that patients who are unable to get to the Surgery when our doors open at 6.45am are not penalised and is a much fairer process.
GP Access Hubs
We have a new service within our locality called GP Access Hubs. This is based at Maywood Health Centre, Hawthorn Road, Bognor Regis and Avisford Medical Group (Yapton Surgery). Each of our practices in our Regis locality has access to these slots and they are available throughout the week until 8pm including some Saturday's and Sunday's (subject to clinical cover). Appointments are strictly for new problems and not follow-ups for any pre-existing conditions. All patients will have to be able to get to Maywood Health Centre or Avisford's Yapton Surgery and they will have to give their consent for us to share their medical record with the clinician that they see. For more details on the criteria for access to this service please contact our reception staff.

As most patients are now aware, the Practice is currently offering online services to enable patients to book & manage their appointments online, & also the facility to order their repeat prescriptions for medicines & appliances using Systmonline.
We plan to extend this online facility to allow patients to view & print summary information from their patient record relating to medicines, allergies, adverse reactions & any other items which may be agreed between the Practice & the individual patient. You are also able to view your "Detailed Coded Records" which has now become available. Please contact the Practice for more details.
The Practice has introduced the use of GP2GP messaging technology during the last year, this enables the Practice to receive patient clinical records electronically when new patients join the Practice list.Further information on GP2GP can be found at: https://systems.hscic.gov.uk/gp2gp
Summary Care Records
The Practice intends to start using the new Summary Care Record facility during 2016. A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from & previous adverse reactions to medication. Having this information stored in one place makes it easier for healthcare staff across the NHS to treat you in an emergency, or when your GP practice is closed.

(Patients who have previously opted out of the Summary Care Record will not have their details uploaded. Should you wish to withhold your records please complete an opt-out form & send it to the practice & we will update your record accordingly. You may change your mind regarding your preference at any time by informing the practice).
Family and Friends Test
We would like your feedback on the care or treatment we give you any time you visit your GP or have contact with the practice. It doesn't take long. Put us to the test and tell us what is working and what we can improve. You can say what you think without giving your name and we will use the information to plan improvements to our services. Named GP Information
You may be aware that all practice are now required to provide their patients with the name of a GP who will have overall responsibility for the care and support that their surgery provides to them. At our practice every patient is given a named or "usual" GP at the time they register, so nothing has changed for our patients.
Although your named GP will have overall responsibility for the care and support that our surgery provides to you, this does not prevent you from seeing any of the GPs in the practice in exactly the same way as you have in the past. We should like to reassure you that you may continue to access all our services in exactly the same way as before. Should you visit a doctor, hospital or care provider from outside the practice you may give them the details of the GP you usually see, you do not have to give them the details of your named GP. Likewise, any communications we receive from outside the practice, such as hospital letters or results, will be directed to the attention of the GP you usually see. You do not need to take further action, but if you have any questions or concerns please feel free to contact the practice. Should you wish to change your named GP details please contact the practice and we will aim to accommodate your request.
