How to Register
Check you live within the boundary area
The practice area extends from the Butlins roundabout and east of Upper Bognor Road to include Felpham, Middleton on Sea and Elmer and embraces Yapton Road up to Bilsham Corner Junction.
Complete a New Patient Registration Form
Registering is simple and takes a few minutes. You will be asked to fill in the online form below. You do not need proof of address or immigration status, ID or an NHS number.
You will be asked to make an appointment with our nurse for a new patient medical, which includes taking details of your past medical history and a check of your current medication – please remember to bring all medication you are currently taking with you to the appointment. The nurse will also check your blood pressure, height & weight, and discuss smoking/drinking, any family history of note, and, for women, take details of the last smear / mammography. It would also be useful if you could bring a urine sample, in a small clean bottle, for testing.
Temporary Services
If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.
To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
Your Named GP
Every patient has a named GP. As a patient, you will be allocated to one of the GPs but please note that you have the option to see any GP you wish.
